The purpose of this screening is to determine whether chiropractic care is appropriate or whether another form of medical care is needed first.

Responsible chiropractic care means knowing when to treat, when to modify care, and when not to treat.

Symptoms That Require Emergency Medical Care

Some symptoms may indicate a serious medical condition that requires immediate emergency evaluation, not chiropractic treatment.

Seek emergency care right away if you experience:

  • crushing or severe chest pain
    • difficulty breathing
    • sudden facial drooping or slurred speech
    • sudden weakness in an arm or leg
    • loss of consciousness
    • severe trauma or suspected fracture
    • loss of bowel or bladder control
    • saddle anesthesia, or loss of sensation in the groin area
    • a sudden, severe headache described as the worst headache of your life

These symptoms may indicate conditions such as stroke, spinal cord compression, pulmonary embolism, or heart attack.

This list is not exhaustive. When symptoms are severe, sudden, unusual, or concerning, emergency medical care should be sought.

Situations That May Require Medical Evaluation First

There are also situations where chiropractic care may still be helpful later, but additional medical evaluation should occur first.

These may include:

  • recent significant trauma
    • suspected fractures
    • serious neurological symptoms
    • infections or systemic illness
    • unexplained swelling in a limb
    • signs of blood clots
    • certain tumors or cancers
    • severe spinal instability

Depending on the history and examination, imaging studies such as X-rays, MRI, or CT scans may be recommended before chiropractic care begins.

Examples of Responsible Screening

Examples include patients who presented with symptoms later identified as or concerning for:

  • pulmonary embolism
    • hypertensive crisis
    • possible acute coronary syndrome
    • suspected deep vein thrombosis, or DVT

In these situations, patients were directed to emergency medical care instead of receiving chiropractic treatment.

Recognizing warning signs is an important part of responsible chiropractic practice.

Blood Clot and Circulatory Concerns

Certain factors may increase the risk of blood clots and require careful evaluation, including:

  • recent surgery
    • prolonged immobilization or casting
    • long flights or extended travel
    • hormonal medications such as birth control
    • unexplained swelling in one leg

When these risk factors are present, patients may be referred for medical evaluation before chiropractic care is considered.

Neurological Red Flags

Certain neurological symptoms require careful evaluation before treatment, especially when they appear suddenly or worsen rapidly.

These include:

  • significant muscle weakness
    • progressive numbness
    • loss of coordination
    • changes in balance or walking
    • difficulty speaking or swallowing

When symptoms are sudden, progressive, or severe, medical evaluation should occur immediately.
